If your GP has issued a Chronic Disease Management (CDM) plan or Team Care Arrangement (TCA), bring the referral paperwork and your Medicare card — this may entitle you to a Medicare rebate on up to five allied health visits per calendar year.

What should I bring to my first appointment?

Please wear or bring clothing that allows easy access to the area being assessed — shorts for knee or lower limb concerns, short socks for foot assessments. Bring the shoes you wear most often, including runners or sports shoes if you’re being seen for a sports or running injury. Also bring your Medicare card and private health insurance card (or app). If you have any relevant imaging — X-rays or MRI reports — bring those along too.
